Patient Care Teams (PCTs) work with your family doctor and consist of mental health professionals and nurses.
The objective of PCTs is to increase the emphasis on chronic disease management, health promotion, disease and injury prevention and improved care of medically complex and chronically ill patients. If your family physician is a member of the CWC PCN, they may work with these professionals to provide enhanced patient care. Availability varies by clinic.
